To partially sponsor a piece of equipment, please choose the amount you wish to donate.

A plaque with the donor's name and dedication memo will only be affixed by the equipment that have been fully-sponsored. 
Please clearly write the following:
  1. Name of Donor (As you wish it to appear on the plaque)
  2. The equipment(s) you are sponsoring 
  3. The dedication memo you wish inscribed on the plaque (in the honor of, for the Illuy Neshama of, etc) (35 words max)
